Shooting at ISO 400/800/1600 vs ISO 100 Pushed (Dolls)

Shooting at Base ISO 100 with ideal ETTR exposure is always preferred, but shooting at higher ISO can become necessary, e.g., handheld shooting and/or keeping exposure times (even on a tripod) from becoming too lengthy.

This page evaluates whether it is better to raise the ISO in camera versus shooting at Base ISO 100 with a push during raw conversion. Which approach delivers the best image quality?
