How to Organize Images on Disk and for Lightroom, Aperture, etc

I regularly field questions in email about organizing images. Indeed, approaching a program such as Adobe Lightroom is daunting for anyone starting out and can generate a headache downstream if some thought is not given to organization, storage, etc.

My own workflow needs do not involve Lightroom, but any Lightroom or Aperture or similar user would do well to consider a two-layer organization that first makes it well organized with hardly any effort, then layers on Lightroom or Aperture, etc.
